complete our patient screening form prior to their appointment to ensure the health and safety of our patients and staff. If you answer yes to any of the questions below, we kindly request to reschedule your appointment to a future date.

    Are you (or the patient if a minor) experiencing any of the following new or worsening symptoms?

    In the last 14 days, have you (or the patient if a minor) returned from travel outside Canada, including the United States, and been directed to quarantine?

    Have you (or the patient if a minor) been identified as a close contact of a COVID-19 positive case by Public Health?

    Have you (or the patient if a minor) been told to self-isolate by Public Health or a Health Care Professional?
